Skilled Nursing (RN/LVN): Medication management, disease education, post-surgical care, wound/ostomy care, catheter & IV care, monitoring and coordination with your physician.
Physical Therapy (PT): Strength, balance, gait training, mobility and fall-prevention to restore function and confidence.
Occupational Therapy (OT): Daily-living skills, home safety, adaptive strategies, and energy conservation.
Speech Therapy (ST): Swallowing therapy, speech/language, cognitive-communication rehab.
Medical Social Worker (MSW): Community resources, counseling, advance care planning, and care coordination.
Certified Home Health Aide (CHHA): Personal care, hygiene, safe transfers, and support with daily routines.
Referrals & Intake
We accept referrals through Care Port, Ensocare, Aiden, & Aida.
Providers can also fax orders to (818)886-8001 or call (818)330-7003 for rapid intake.
Documentation we handle: physician orders, face-to-face certification, discharge summaries, and care coordination.
